Hernia through foramen of Bochdalek is the most common congenital diaphragmatic hernia. Although quite common in infants and children, this lesion is rarely reported in the adult. When symptom develop in the adult, they are different from infantile type in the aspect of clinical symptomatology and prognosis. We experienced a case of Bochdalek hernia in an adult. This 34 year old housewife was transferred to the Department of .Internal Medicine, Severance Hospital with complaint of dull and intermittent left sided chest pain of 5 days duration. She had no history of trauma or severe abdominal symptom. The initial chest roentgenogram revealed hazy infiltration with questionable gas shadow suggesting pleural effusion and basal pneumonia. Barium enema study confirmed left diaphragmatic hernia and Bochdalek hernia was confirmed by operation.
